Step 1 - Go to your account dashboard and look for the Patient Activation Measure (PAM) tool

Step 2 - If you are setting up PAM for the first time, the tool will be locked.  Select the 'Complete assessment' button. 

Step 3 - Now complete the assessment questionnaire 

Step 4 - You will now be graded an activation score, and redirected to the tool on your dashboard

Step 6 - NowPatient will now set your health support and education resources to reflect your activation score.  If you have a low activation, we will engage with you more frequently with healthcare support and clinical interventions.

Step 7 - Every 3 months, you will be prompted to repeat the assessment.  As you get more engaged, the graph will change to reflect your new updated score.
