What is the NowPatient Hospital Admissions Reporting service?
Hospital Admissions Reporting is a proactive health monitoring service that helps you track your hospital visits and stays while contributing to wider medical research. Every three months, NowPatient will prompt you to report any hospital admissions you may have experienced. This creates a comprehensive record of your healthcare utilisation, helping you understand patterns in your health and communicate more effectively with your healthcare providers. Your contributions support healthcare planning, medical research, and health system improvements, while also providing you with personalised insights into your health journey over time.
Can I report hospital admissions outside the quarterly cycle?
Yes. In addition to the quarterly reminder, you can make ad-hoc reports at any time — for example, to document an admission while the details are fresh in your memory.
Is Hospital Admissions Reporting free?
Yes. The Hospital Admissions Reporting service is free for all registered NowPatient users.
How does the reporting process work?
The reporting process is designed to be quick and straightforward. You will be asked about when you were admitted, the reason for your admission, how long you stayed, and the outcome of your visit. This information is securely stored in your personal health record and can be reviewed at any time.
How does the quarterly reminder work?
NowPatient sends you a reminder notification every three months to prompt you to report any hospital admissions you may have had. Follow the instructions on the notification and select the button that applies to you: ‘Report Hospital Admission’ or ‘No Quarterly Hospital Admissions to Report’. If you select ‘Report Hospital Admission’, you will be taken to the report page to complete your submission.
How do I report an ad-hoc or unplanned hospital admission?
You can submit an ad-hoc hospital admission report at any time. To do so:
Navigate to the ‘Report Hospital Admissions’ Service Card in the Health Surveillance section of your account homepage.
From the Options menu, select ‘Report Hospital Admission’.
You will be prompted with a series of questions to complete your report.
Once completed, you do not need to take any further action.
Do I have to wait for the quarterly reminder to report an admission?
No. While NowPatient sends a quarterly reminder, you can submit a report at any time. Reporting soon after an admission ensures the details are accurate and your record remains complete.
How do I view my historical hospital admission reports?
To view your past reports:
Navigate to the ‘Report Hospital Admissions’ Service Card in the Health Surveillance section of your account homepage.
From the Options menu, select ‘View Hospital Admissions’.
Here you can view all the historical unplanned hospital admission reports you have made. You can open them individually to view.
Can I share my hospital admission reports with my doctor?
Yes. You can generate and download an aggregate report to your documents folder to share with your Clinician. To do so, use the date range selector to select the period you want, choose the reports you wish to export, and click ‘Export’. The aggregate report will be sent to your documents folder, from where you can share it with your Clinician.
What types of hospital admission should I report?
Report any hospital admission, including:
Emergency department visits that led to admission
Planned or scheduled hospital admissions
Surgical procedures requiring hospital stay
Observation stays
Intensive care unit admissions
Transfers between hospitals
What details will I need to provide for each admission?
For each admission, you will be asked to provide:
Date of admission and discharge
Reason for admission (diagnosis or symptoms)
Name of hospital or healthcare facility
Length of stay
Any procedures or surgeries performed
Outcome (fully recovered, ongoing treatment, etc.)
How does regular reporting benefit me personally?
Regular hospital admissions reporting helps you in several important ways:
Track your health patterns and trends over time, identifying risk factors and opportunities for preventive care.
Build a complete medical history that is invaluable when seeing new doctors, managing chronic conditions, or coordinating care across multiple providers.
Monitor whether your health is improving, stable, or requires additional attention, and identify seasonal patterns or triggers.
Receive personalised insights, including recommendations for preventive care and strategies to help you stay healthier and avoid unnecessary hospitalisations.
Have precise dates, diagnoses, and outcomes readily available for more productive conversations with your healthcare providers.
Identify opportunities for prevention and early intervention, helping your care team adjust your treatment plan if you are being admitted frequently.
Coordinate care more effectively, reducing the risk of conflicting treatments or missed information during transitions of care.
What happens when I log an unplanned hospital admission?
When you log an unplanned hospital visit, NowPatient will check whether you have completed a Patient Engagement Score assessment. This assessment measures how actively you are managing your health. Your score helps NowPatient personalise its support to match your understanding of your condition. With the right tools and tailored guidance, NowPatient can help you avoid preventable hospital visits in the future.
How does my reporting benefit the wider healthcare community?
Your reports contribute to wider healthcare improvements:
Healthcare planning and resource allocation — helping health systems understand patient needs and plan staffing and resources effectively.
Medical research into disease progression, treatment effectiveness, and factors that lead to hospitalisation.
Identifying seasonal patterns, emerging health threats, and gaps in outpatient care.
Developing better outpatient services, chronic disease management programmes, and interventions that help patients stay healthy at home.
Better care coordination, improved discharge planning, and reduced unnecessary hospitalisations for everyone.
Identifying communities or populations with inadequate access to preventive care, enabling targeted improvements in healthcare equity.
Is my data private and secure?
Yes. Your hospital admission reports are stored securely in your personal NowPatient account and are protected by healthcare-grade encryption in line with Information Governance and HIPAA protocols. Your personal information is never shared without your explicit consent.
